Familial Hypercholesterolaemia Nurse

We are excited to invite applications for Specialist Familial Hypercholesterolaemia (FH) Nurses to join the Yorkshire & Humber Familial Hypercholesterolaemia Service – a dynamic, innovative, and region‑wide nurse‑led service dedicated to improving the diagnosis and management of adults and children with Familial Hypercholesterolaemia and associated lipid disorders.
About the Service
The Yorkshire & Humber FH Service provides a comprehensive tertiary‑level, nurse‑led provision across the region. It is hosted by York & Scarborough NHS Foundation Trust, acting as the Lead Provider for a collaborative network that includes:
* Leeds Teaching Hospitals NHS Trust
* Hull University Teaching Hospitals NHS Trust
Our FH nurses deliver expert screening, treatment, and specialist advice to support patients and families living with FH across the region. The role is based at Hull University Teaching Hospital.
Main Duties
The post holder will provide specialist nursing advice and support to patients referred into the familial hypercholesterolaemia (FH) pathway within their local provider site. Responsibilities include:
* Delivering a nurse‑led service to identify index cases and coordinate family screening for both adults and children.
* Working collaboratively with local and regional multidisciplinary teams to contribute to all aspects of the FH service, including pathway delivery, service set‑up, monitoring outcomes, quality improvement, and producing reports for stakeholders.
* Demonstrating autonomy, professional leadership and initiative to drive ongoing service development.
* Using advanced clinical, educational and management skills to ensure a seamless, patient‑centred service across organisational and professional boundaries.
* Actively engaging with MDTs, primary care and national partners to support evidence‑based practice, education, research and audit.
Person Specification
Essential
* Registered Nurse
* Minimum 12 months experience in FH
* Degree‑level education or demonstrable skills to this level
* Ability to manage clinic lists and review patients independently
* Non‑Medical Prescriber
* FH Pediatric Trained
Desirable
* Strategic Clinical Networking Experience
* Experience of setting up a service
