This position will see you predominantly out in the field, working side by side with our clients and using your specialist expertise in case management to proactively achieve successful tenant outcomes.

What will your new day look like?

* You will be responsible for overseeing a designated caseload, handling everything from the initial referral to intake and eventual exit.
* You will actively engage in daily face-to-face client meetings, promptly addressing client emergencies in real time and ensuring positive tenant outcomes through follow-up actions.
* Develop and implement collaborative individual support plans with residents that include a range of interventions and address the needs and goals of the client.
* Proactive seek feedback from relevant support partners and follow up support partners weekly to assess client’s needs and create additional strategies to optimise client support.
* Deliver wholistic proactive, hands-on support to clients, which may include accompanying them to appointments and providing support during these appointments.
* Actively participate and be present at relevant block meetings, resident engagement activities, and new housing sites to raise awareness of support service unit offerings and the referral process among residents.
* Maintain and nurture networks and relationships with external support agencies and community organisations.
* Proactively gather feedback from relevant support partners and conduct weekly follow-ups to assess client needs, devising additional strategies to enhance client support.
* Closely monitor the progress and outcomes of individual case plans, ensuring that the plans continuously align with client needs and goals, including coordinating timely reviews, PWIs, surveys, and exit planning.
* Engage effectively with internal departments and particularly with Housing Services on complex residents and identify collaborative strategies to help sustain tenancies.
* Work with partners to understand client outcomes and ensure client engagement.
* Promote the Support Services and associated programs both internally to staff and externally to potential clients.
* Maintain up-to-date documentation and files within a timely manner in database system and any other data capturing tools.

Some important things you will need to begin this journey!

* Degree qualified in Human & Social sciences e.g. social work, mental health, community development services, social work.
* A minimum of three (3) years of hands-on experience in case management, involving clients facing diverse and intricate challenges, including, but not limited to, mental health issues, domestic violence situations, and experiences related to hoarding and squalor.
* Proven ability to engage and support individuals with challenging needs and evidence of processes undertaken to develop and implement comprehensive client support plans.
* Strong knowledge and understanding of underlying causes of homelessness and complex needs.
* Flexibility to respond to unplanned client needs requiring face to face field support work on a daily basis.
* Demonstrated application of effective and appropriate written and verbal communication skills.
* Demonstrated understanding, respect and sensitivity towards cultural and social diversity and clients with complex needs, especially individuals from Aboriginal and Torres Strait Islander and culturally and linguistically diverse backgrounds or communities.
* Proficient level of experience in using and learning software applications (SRS, Office 365 & Excel) and any other office technology
* Knowledge of Government policy and legislation applying to community housing.
* Proven experience and knowledge of Trauma Informed Practices
* Possess a current Drivers Licence
* Willingness to obtain a National Criminal Record Check
* Willingness to obtain a NDIS check
