You will be providing opportunities for patients to engage in a wide range of leisure and social activities on the ward and one-to-one and group sessions in line with their rehab goals and their hobbies, interests, and personal values and identity, using a planned weekly schedule. Main duties of the job To assist patients in identifying their preferred leisure / social activities using information from patients, families and utilising assessment tools as appropriate. To encourage and support patients to take part in activities suited to their particular interests and needs. To liaise with relevant therapists regarding patient goal areas, appropriate activities, and any relevant recommended strategies or recommendations To plan, organise and provide a range of individual and group activities relevant to the patientsu2019 changing needs and in response to patient choice. This will include arrangement of location, preparation of resources and access to funding / transport if required. To maintain a patient-focused approach at all times, by ensuring that activities are suited to the personu2019s specific preferences, cultural beliefs, and needs, and adapting them flexibly as needed. To promote the service, encourage patient engagement and work flexibly alongside the team to obtain maximum uptake. To support and encourage patients to access the local community. To evaluate patient satisfaction and participation through a range of feedback measures. To provide information on available resources within the local community and from associated agencies in conjunction with therapists. Person specification Education Essential criteria Functional Skills Level 1 in Numeracy and Literacy - GCSEs Maths and English Grades A-E or 9-2. Or equivalent. Desirable criteria NVQ level 2 or equivalent or willing to work towards diploma level 2. Experience and Skills Essential criteria Previous customer service experience. Ability to organize and manage day-to-day departmental activities and the activities of large multidisciplinary team within this. Communication skills u2013 able to relate and communicate with patients and staff at all levels and from different professions.
