Position description / Job description Conduct comprehensive assessments of clients' needs and develop individualized care plans. Provide direct social work services to individuals and families. Collaborate with multidisciplinary teams to ensure holistic client care. Advocate for clients' rights and access to resources. Maintain accurate and timely documentation of client interactions and progress. Participate in case conferences and supervision sessions. Develop and facilitate support groups or workshops. Engage in community outreach and education initiatives. Requirements We're looking for you, if: You hold a relevant Social Work degree (e.g., Bachelor of Social Work). You possess a valid Social Work license or registration. You have a minimum of 2 years' post-qualification experience in mental health social work. You possess excellent communication, interpersonal, and problem-solving skills. You demonstrate cultural sensitivity and the ability to work with diverse populations. You are proficient in using electronic health records and other relevant software. Experience working with adults with mental health issues is an advantage.
