Job Overview
This role is within primary care, part of the community mental health team in Denbigh.
We conduct assessments from various referring agencies through our SPOAA daily meetings and provide brief interventions in group settings and occasionally one-on-one.
Familiarity with third sector agencies is beneficial as we aim to strengthen relationships and improve signposting to these valuable resources.
Main duties of the job
An exciting opportunity has arisen in Primary Mental Health Support services for an experienced, motivated, and forward-thinking practitioner to join a busy, supportive team in Denbighshire.
The successful candidate will work closely with a multidisciplinary team, receiving referrals directly from GPs, providing short-term interventions for individuals with mild to moderate mental health issues, and collaborating with secondary care, specialist services, third sector organizations, and statutory agencies.
They will manage a caseload, develop risk management and care plans based on thorough assessments, and demonstrate excellent communication, prioritization, and time management skills.
Knowledge of Welsh is desirable; applications from Welsh and/or English speakers are equally welcome.
