What you will do:
* Manage a complex workload, assessing and planning support for individuals with intricate needs, and coordinate outcomes to encourage independence.
* Conduct assessments using health and social care tools, including contributions to continuing healthcare evaluations.
* Identify reablement and enablement opportunities, facilitating access to these services.
* Implement a strengths-based approach to promote independence and wellness, enabling people to assess their own needs, risks, and goals, delaying long-term care dependency.
* Recognise and utilise community resources and individual assets aligning with the Adult Care Strategy.
* Regularly monitor and review service provisions, ensuring coordination and consideration of all alternative solutions with stakeholders.
* Engage and negotiate with local service providers for enhanced outcomes.
* Offer expert advice and support to clients, their carers, and colleagues, forging strong relationships and partnerships.
* Fulfil safeguarding duties as a Safeguarding Officer.
* Maintain professional standards and accountability for your performance and the quality of work.
* Keep managers informed about any issues impacting quality, budget, or performance.
* Assist in achieving service targets and maintain efficient partnership arrangements.
* Foster a supportive and safe working environment, upholding legislation and policy.
* Perform duties as an Approved Adult Mental Health Practitioner, Best Interests Assessor, or Practice Educator when appropriately trained.
* Contribute to ongoing practice and service development, advising, mentoring, and guiding less experienced staff.
* Organise and manage your workload independently with guidance from senior practitioners.
