Salary and Hours
Salary £15,218 + on call payments. Driving licence required. Monday to Friday 9am – 1:30pm with additional on call duties on a rota basis.
Job Description
What you’ll do
* Coordinate care: manage referrals, create care worker rotas, liaise with professionals and monitor service users’ conditions.
* Staff management: supervise care workers, conduct appraisals, maintain performance standards.

Qualifications
Experience & organisation: efficient, organised Care Coordinator passionate about community care. Proven ability to manage rotas to impeccable standards and build strong relationships with care workers.
Additional requirements: previous healthcare experience and solid understanding of CQC, care plans and rotas.
