Job Overview
An exciting opportunity for a Band 3 Health Care Support Worker to join the Hospice at Home Team Evening Service. This 7‑day, part‑time permanent role covers 16 hours of service per week, primarily 6pm‑10pm, with occasional shift extensions.
Key Responsibilities
* Provide high standards of care to patients at end of life in their homes.
* Offer emotional support to patients’ families and carers.
* Maintain privacy, respect, dignity and confidentiality at all times.
* Communicate effectively with patients, families and multidisciplinary teams.
* Carry out basic nursing care and holistic support as directed by the Team Leader.
* Co‑ordinate with agencies such as Macmillan, District Nursing, Marie Curie and hospices to improve end‑of‑life care.
* Work rota‑d weekends and Bank Holidays as required.
Working Conditions
* Contract: 16 hours per week, part‑time, permanent.
* Shift: 6pm – 10pm, 7 days a week, with occasional shift changes.
* Location: Wrexham Maelor Hospital, Wrexham, Flintshire, and Denbighshire county homes.
* Vehicle access required to travel between patients’ homes in towns and rural areas.
Qualifications & Experience
* Essential criteria: QCF/NVQ Level 3 Health Care qualification or equivalent; experience working in a care setting; ability to work evenings and weekends.
* Desirable criteria: Palliative care training and/or education; experience working with palliative care patients; Welsh language skills.
