What you'll do (Key Duties):
* Key Working: Manage, compile, and review individual care plans.
* Direct Care: Provide personal and intimate care with dignity.
* Support: Assist with equipment, promote mental/physical activity, and ensure a high standard of living environment and personal effects.
* Liaison: Work closely with health professionals, especially during illness or end-of-life care.
* Documentation: Maintain accurate records and reports in line with SSSC standards.
* Commitment: Work on a rota basis, including evenings and weekends, and participate in meetings and training.
