Band 5 - Children's Community Nurse - 34.5 hours per week
The Children’s Community Nursing Team is an integral part of the Ambulatory Model offered within the Tameside and Glossop locality. The team accepts direct referrals from the emergency department, children’s observation and assessment unit, children’s ward, and local GPs, working to facilitate early discharge from hospital and providing a rapid response service to prevent unnecessary hospital attendance or admission.
The team offers nurse-led clinics to ensure children and young people with non-acute conditions receive care close to home.
You must be a registered children’s nurse with extensive clinical knowledge and skills in caring for children and young people with various needs in acute services, enabling autonomous practice and sound clinical decision-making.
Experience in community settings is desirable. You should have excellent interpersonal and communication skills and demonstrate sound, evidence-based theoretical knowledge.
You should be committed to delivering high standards of care and capable of working independently and within a multidisciplinary team. The role involves significant lone working, supported by robust safeguarding policies for Children’s Community Nurses.
