Job Summary:
We are looking for an enthusiastic and proactive Clinical Pharmacist to enhance our clinical team. You will work closely with GPs, nurses and other health care professionals to improve medicines optimisation, provide structured medication reviews, and support safer, evidence-based prescribing.
This is an exciting opportunity for a pharmacist who is passionate about improving patient care and keen to develop their clinical role in a welcoming seaside practice.
Main duties of the job
Summary of Duties
* Deliver structured medication reviews, particularly for patients with long-term conditions or on multiple medications.
* Ensure safe, effective, and cost-efficient prescribing, including monitoring high-risk medicines and managing repeat prescriptions.
* Conduct patient consultations (in person or remotely) and support deprescribing where appropriate.
* If an independent prescriber, manage minor ailments and chronic conditions within scope of practice.
* Lead on medicines optimisation, audits, QOF, and prescribing incentive schemes.
* Collaborate with community pharmacies, care homes, and NHS teams to ensure seamless care.
* Provide education and training to staff, keep up to date with guidance, and mentor pharmacy team members when needed.
