Nurse Practitioner (Care Coordination, Oncology & Out of Hours Triage) – NHS West Midlands, England, United Kingdom
Job Summary
Shropdoc is a not-for-profit provider delivering urgent primary care services across Powys. Core services include Powys Out of Hours (OOH) GP care, urgent care triage, home visiting, face-to-face clinical services, Care Co-ordination Services and Oncology Helpline.
The post is linked to all work streams and services across the organisation, supporting a coordinated and integrated approach to patient care. The Nurse Practitioner will provide advanced, autonomous, and person-centred care within a 24/7 integrated triage and coordination service.
Responsibilities
* Deliver safe and effective triage and clinical assessment services via telephone and digital platforms both in-hours and out-of-hours, accurately collating information using electronic clinical systems and determining the most appropriate care response or escalation.
* Independently assess, interpret, and synthesise clinical information to make autonomous decisions, avoiding unnecessary onward referral where safe to do so.
* Provide an evidence-based, nurse-led Oncology and Haematology Helpline managing chemotherapy-related toxicities, treatment side effects, and life-threatening symptoms, with a holistic focus.
* Act as a Care Coordination Centre clinician, liaising with referrers and care teams to agree and implement care plans, ensuring safe navigation through complex pathways.
* Provide health promotion, self-management education, and personalised care planning with patients and carers to support autonomy and wellbeing.
* Support safeguarding responsibilities and escalate concerns in line with organisational policy.
* Document all patient contact and clinical decisions in electronic systems, maintaining professional standards and confidentiality.
* Maintain professional standards, registration, and participate in clinical supervision and mentorship.
Qualifications and Experience
* Essential: Professional registration (NMC); experience of multi-disciplinary working; excellent communication, reasoning and interpersonal skills; ability to perform holistic assessment and make clinical judgements; ability to work independently within occupational guidelines; skilled in clinical history taking and accurate recording of notes.
* Desirable: HCPC considered if previous NMC registration; experience delivering patient care in urgent care, primary care, or community settings; ability to liaise with multi-disciplinary teams; computer literacy and ability to type patient notes on the phone.
Salary and Benefits
Salary: £23.55 per hour with uplifts (8pm-6am weekdays & Saturdays 30%; Sundays and Bank Holidays 60%). Permanent, full-time with flexible working. NHS Pension Scheme, cycle to work, annual leave 25 days rising to 27, sick pay, Employee Assistance Programme, compassionate leave.
