About the Team
ASCP is a multi-disciplinary, responsive service. We visit individuals in their own homes, understand their circumstances to provide practical solutions – from equipment to short‑term support – helping people regain skills and confidence.
You’ll work alongside an incredible team of professionals, including:
* Social Workers and Social Care Assessment Officers
* Physiotherapists
* Falls Prevention Coordinators
* Moving & Handling Specialists
* Reablement Practitioners
* Health colleagues
Together we deliver rapid interventions that prevent unnecessary hospital admissions and help people stay independent for longer.
About the Role
As an Occupational Therapist, you’ll:
* Support people to maintain independence and avoid unnecessary admissions
* Contribute clinical expertise to a team tackling falls, frailty, long‑term conditions and unexpected crisis
* Ensure timely, strengths‑based interventions. This is a front‑facing role where your decisions matter. You’ll make an impact from the very first conversation, shaping care pathways and helping people live safely and confidently in their own homes.
