We have a brand new exciting opportunity for an individual to join our PCLN Team working with adults with Learning disabilities.
Responsibilities
* The Health Liaison worker will work as part of the Primary Care Liaison (PCLN) Team and support the wider Liaison Teams to identify those patients that are eligible for an annual health check and collate information to enable access to the wider IDD Services via the IDD SPA.
* The Health Liaison worker will have a specific remit to support the Primary care liaison team to engage service users in accessing primary care services to improve their physical health.
* Support the PCLN Team in engaging with those patients that have not attended for their annual health checks; this may include contacting them and their families and encouraging attendance at health and screening appointments.
* The Health Liaison Worker will support the PCLN Team with the administration around screening projects.
* Support the primary care liaison nurses with the verification process and share information with IDD SPA Colleagues.
* To undertake a range of delegated tasks within their role to support the IDD Liaison Teams and SPA (single point of access to IDD Services) – e.g., attend the SPA MDT meetings to confirm eligibility for IDD services.
* To have a good working knowledge of both SystmOne and Rio and other patient recording systems.
* To navigate IT Systems and manage patient information.
* To provide care, under basic healthcare checks (use of wheelchair scales), screening processes when competent to do so, and support to service users and their carers in the community setting (taking them to appointments where appropriate).
* Support and promote reasonable adjustments for patients accessing primary health settings.
* Encourage the documentation of reasonable adjustments and share known adjustments within the patient record.
* To support the Primary care liaison nurses to follow up patients who DNA or were not brought to appointments to ascertain what the barriers are for non‑attendance.
* To report and record the death of a person with an IDD in line with trust policy and LeDeR guidance.
* To support the primary care liaison team in investigating queries in relation to eligibility for inclusion on the GP Register and the sharing of information with colleagues within the wider liaison team and IDD SPA.
* To respond appropriately to urgent and non‑routine situations as delegated.
* To participate in team meetings to contribute to the development and effectiveness of the service.
* To ensure confidentiality is always maintained.
* To work independently and as part of a team.
* Support the Enhanced Service verification process with PCLN Colleagues.
* Signpost patients to supportive physical health interventions accessible to the service user.
* To work with the wider MDT and provide feedback on patient care.
* To take an active role in assisting patients to take care of their physical health and understand the importance of good physical health in relation to their mental health.
* Under the guidance of the Qualified Nurse, liaise with the wider MDT and health community when advocating for individual needs of the patient.
* Support with the National Screening programmes and assist the PCLN Nurses with bowel screening calls and similar screening work.
* Support with blood desensitization work.
* Undertake physical health screening where appropriate – e.g., weighing using electronic wheelchair scales and basic observations as required and record and report back to the Qualified Nurse.
* Assist with collating patient feedback and letters to patients.
* To undertake any other reasonable duties as requested by the Team Leader.
* The role will include working across different bases across the city and county of Nottinghamshire and having a base within Clifton Cornerstone.
* The role will include some lone working.
* The post holder will at times visit patients within their own home environments and other community settings.
* The post holder will work under the direction of the Qualified Nurse.
