Band 5 Physiotherapist- Community Rehabilitation Service.
An exciting opportunity has arisen for a part time Physiotherapist position within Mendip CRS working 27.5hrs/week. This post will be primarily based in Frome in-reaching to the ward and supporting the community.

We are looking for dynamic and inspiring Physiotherapist to join the Community Rehabilitation Service in Mendip, you may be required to work across the 3 main Mendip bases.
You will gain an in depth knowledge and understanding of community working and supporting people within their own homes. You will be facilitating people to identify their goals on which to base and progress their individual rehabilitation programme.
Successful candidates will be directly supported by the CRS team and Clinical Specialist within the team.
Main duties, tasks & skills required
You will be expected to carry out specific therapy assessments which will include goal setting and treatment planning, assessing risk, promoting and enabling independent functioning and progression. You will be committed to a person-centred approach to your work, have an ability to prioritise and work flexibly. Working alongside other provider teams and services.
You will be working alongside team members which include OTs, Physios, Rehab Assistants and Assistant Practitioners and Pharmacy Technician. You will be delegating rehabilitation tasks and over-seeing rehab programmes for individuals.
We run an in-service training programme and peer group learning.
A car driver is desirable.

Essential

Degree/Diploma/Masters level education
Registration by the Health Professions Council (UK) as a Physiotherapist.
Desirable
Evidence of CPD maintained in a portfolio including attendance at relevant post grad courses.
Post graduate accredited course in Care / rehabilitation of the Elderly and/or other relevant post graduate courses
Experience
Essential
Proven post graduate experience with extensive clinical experience relevant to the post.
Experience of a wide range of clinical rehabilitation areas of practice to include elderly patients with complex multi-pathology, neurological, orthopaedics etc
Experience of working as part of a multidisciplinary team
Desirable
Previous Community Rehabilitation experience
