Duties
* Working with adults who have complex social care needs whilst carrying a varied caseload with increasing complexity and autonomy.
* Contribute to safeguarding, assessments, care planning, and service delivery in collaboration with health and care partners.
* Play a vital role in promoting personalisation, ensuring services are shaped around the individual, supporting independence and enabling adults to live fulfilling lives in a strengths‑based way.
* Work collaboratively with individuals, their families, providers and other health care professionals, to proactively encourage independent and safe living within the community.
