Overview

We have an exciting opportunity to recruit Occupational Therapy and Physiotherapy Advanced Practitioners to embed the AHP front door service in Accident and Emergency and the admission ward (GA) within Raigmore Hospital. These posts support the continuing development and growth of the AHP front door service, with a focus on early frailty assessment and admission avoidance where appropriate, and setting goals and functional criteria for discharge early in the patient journey.

The successful candidates will demonstrate an in-depth understanding of community services, work in partnership with the MDT at Raigmore and with community integrated teams to ensure patients receive the right care in the right place at the right time. You should be forward thinking and proactive with excellent organisational and clinical skills, good communication, and the ability to make autonomous decisions under pressure. The ability to guide data collection and evaluate the efficiency of the service will be a key skill.

This service will run 12 hour day shifts over 7 days; applications for part time, job share and flexible working patterns are welcomed. Please get in touch to discuss this opportunity.

Responsibilities

* Contribute to embedding and delivering the AHP front door service within A&E and the admission ward to support early frailty assessment and discharge planning.
* Work in partnership with the MDT and community teams to ensure appropriate care is provided in the right place at the right time.
* Lead on data collection, monitor service performance, and evaluate efficiency of the service.
* Demonstrate autonomous decision-making under pressure and provide clinical leadership within the team.
* Participate in service improvement initiatives and adapt to a fast-paced health and social care environment.


Qualifications and Skills

* Forward thinking with excellent organisational and clinical skills.
* Excellent communication abilities and the capability to work autonomously.
* Ability to guide data collection and evaluate service efficiency.


Employment Details

This service operates 12-hour day shifts over 7 days. Applications for part-time, job share, and flexible working patterns are welcomed.
