At Cwm Gwendraeth, we care for 59 adults, offering facilities such as lounges, a coffee lounge, garden spaces, a craft room, and an onsite Social Club called 'Canolfan'. Residents enjoy a hydrotherapy pool, gym, and social spaces for activities and events. Our team is dedicated to maintaining and improving independence through rehabilitation, stimulation, and social integration.
Ty Cwm Gwendraeth is our modern, purpose-built facility offering training programs that support professional growth. We are proud of our multidisciplinary team, including practitioners, therapists, and more, providing tailored care to ensure a high quality of life for the people we support.
Role Responsibilities
* Assessing Care Needs: Evaluate residents' physical, psychological, and social needs and create personalized care plans involving residents, families, and the multidisciplinary team.
* Providing High-Quality Care: Deliver daily care that promotes dignity, independence, and well-being.
* Leading a Multidisciplinary Team: Supervise and support staff, lead shifts, organize duties, and support staff development.
* Monitoring and Reporting: Continually assess care needs, evaluate the care provided, and keep accurate records.
* Promoting Independence and Well-being: Support residents' rehabilitation, social activities, and independence.
* Ensuring a Safe Environment: Maintain safety standards for residents and staff, adhering to policies and regulations.
* Supporting Staff Development: Orient new staff, provide supervision, and facilitate training and career progression.
