Main duties of the job
JobResponsibilities:
To assistand support the practice to provide Structured Medication Reviews and medicineawareness.
To supportthe achievement of the practices prescribing Quality and Outcomes Framework,the PCNs Impact and Investment Fund targets and the locally agreed MedicineOptimisation Scheme.
Tocontribute to the review and development of repeat prescribing protocols/Standard Operating Procedures to improve the efficiency and effectiveness ofrepeat prescribing systems
To provideadvice to GPs, staff and patients in changes to prescribing to support theimprovement of prescribing safety, quality and cost effectiveness
Toparticipate in medication audits
To assistthe Clinical Pharmacist team to input data for the local Medicine OptimisationScheme.
To assistin the appropriate monitoring and management of prescribing the budgets andincentive schemes and to liaise with the Medicines Management team at StaffordshireICB (previous CCG)
To prepareevidence based resources and information to support the implementation ofrational cost-effective prescribing
To take aproactive approach to the safe handling of prescriptions, including assessmentof risk and making recommendations for improvement
To respondto medication queries from patients and staff in a professional mannerreferring to the most appropriate clinician in accordance with practiceprotocols
About us
Primary Care Networks (PCNs) have been created to facilitate delivery of more integrated care. They cover defined geographical areas to enable them to better respond to the needs of local populations and build relationships across wider primary care within natural neighbourhoods. By design they are small enough for relationships to flourish, but large enough to provide a broad scope of services.
Stafford Town PCN consists of 4 practices, Castlefields Surgery, Stafford Health & Wellbeing, Holmcroft Surgery and Wolverhampton Road Surgery.

Dutieswill vary from time to time under the direction of the Partners / BusinessPartner and clinical pharmacist dependent on current and evolving practiceworkload and staffing levels.
StaffManagement:
To provide supervision, training andprofessional leadership to the practice administrative staff as required.
To provide medication review services topatients via remote/ face to face clinics, domiciliary visits and inresidential and nursing homes, and to deliver pharmaceutical care plans thatmaximise cost-effective prescribing and improve the quality of patient care.
To present at patient group meetings or otherappropriate events to give advice on the appropriate use of medicines.
To produce patient information leaflets andposters and run medicine awareness projects throughout the year.
Assist practices with the monitoring andmanagement of their prescribing budgets as appropriate.
To prepare evidence based resources andinformation to support the clinicians and all other relevant healthprofessionals in the implementation of rational cost-effective prescribing.
To assist in planning, development and supportthe introduction of new working processes within the practices and PCN tooptimise the quality of prescribing.

Person Specification
Qualifications
* Completion of a GPhC approved Pharmacy technician qualification as outlined at: https://www.pharmacyregulation.org/education/approved-providers-education-and-training/approved-courses-pharmacy-technician
* Registration with General Pharmaceutical Council of GB
* Maintains clinical and professional competence and CPD
* Completion of CPPE Primary Care Pharmacy Education Pathway
Skills
* Excellent written and verbal communication skills and attention to detail
* Good presentation & organisational skills
* Effective time management and the ability to prioritise
* Ability to work on own or as part of a team and liaise with other professionals
* Ability to plan and manage own workload to meet required deadlines
* Familiar with EMIS Web and Docman GP computer systems
Experience
* Good understanding of general practice and the wider NHS
* Experience of working in a GP Practice or hospital
* Experience to undertake domiciliary home visits as appropriate to undertake medical reviews
