ROLE SUMMARY
Home Care Support Workers at Sincerity Health Care are dedicated to enabling service users to maintain their independence within their own homes. JOB SPECIFICATION
1. Key Responsibilities:
Deliver personalised, high-quality care to service users, following individual support plans and adopting a person-centred approach.
Duties include:
* Personal Care: Assisting with washing, dressing, bathing, toileting, and medication management.
* Social Duties: Preparing and cooking meals, conducting essential cleaning, shopping, and handling bill payments.
* Enabling and Support: Supervising, monitoring, and coaching service users to enhance their independence and self-control. This may involve accompanying service users to appointments, social outings, or other events.
* Understand and respond to the specific needs of service users, maintaining effective communication with them, their families, friends, and other healthcare professionals.
* Ensure all care activities comply with Sincerity Health Care's Policies and Procedures, CQC Fundamental Standards, and all contractual and legislative requirements.
* Maintain accurate and timely records, reporting any changes or concerns in the service user's condition or circumstances.
* Collaborate effectively as part of a team to provide comprehensive support.
* Deliver services in a culturally sensitive manner, respecting the diversity of service users in terms of disability, race, age, sexuality, marital or civil partnership status, transgender status, or religion/belief.
* Work closely with unpaid carers, including family members and friends, to enhance the well-being of service users.
* Uphold anti-discriminatory practices, confidentiality, dignity, and privacy for all service users.
* Act swiftly to safeguard service users from neglect or abuse.
* Engage in staff development, training, and performance appraisals as required.
* Consistently project a professional and caring image, embodying the values of Sincerity Health Care.
