Rehabilitation Assistant (Band 3) – Stroke Supported Discharge Team
Surrey Downs Health & Care is seeking a motivated Rehabilitation Assistant to support stroke patients in their recovery and safe discharge from hospital. The role involves delivering rehabilitation services in patients’ homes and community settings, helping them regain independence and improve their quality of life.
Responsibilities
* Provide rehabilitation support services to the Stroke Supported Discharge pathway.
* Carry out delegated treatment plans and assist with the neuro‑rehabilitation of patients in the community, including inside the department, patients’ homes or other appropriate settings. Treatment may cover Physiotherapy, Occupational Therapy or Speech and Language Therapy.
* Manage own patient caseload; assess and treat routine patients within guidelines agreed by the treating therapist. Monitor progress and make minor alterations to patient treatment programmes as required.
* Work unsupervised while reporting patient progress and informing the treating therapist of any problems. Receive guidance and advice on a near‑daily basis.
