Team Lead Occupational Therapist; Prevention of Admission and Discharge to Assess

Are you an enthusiastic Occupational Therapist ready to lead a community based team? Can you adapt to the challenges of working at the interface of hospital and community, supporting people home from hospital in a timely manner and preventing other people from being admitted to hospital unnecessarily? If so then we’d love you to join our friendly, supportive team.

We are looking for a Team Lead Occupational Therapist to join our city wide service to lead and manage a team of Occupational Therapists and Assistant Practitioners. This is an exciting time to join the Edinburgh Health and Social Care Partnership as we continue to develop both our Home First approach and our AHP portfolio across Edinburgh Health and Social Care Partnership. You will make a vital contribution enabling people to return from home from hospital as soon as medically able, in supporting people to remain at home rather than being admitted to hospital as well as supporting our newly developed Falls pathway.

You will work as part of an integrated team with Physiotherapists, Assistant Practitioners and Community Therapy assistants on a daily basis. You will have excellent organisational skills and be a clear and effective communicator. Previous experience of working in a rehabilitation or community setting with highly developed discharge planning skills are essential.

We are a city wide service and there is an expectation for you to be based in one of four Community bases throughout the city, depending on the priority needs of the service.
