Responsibilities

* The role provider will have responsibility for the organisation of anticoagulation clinics and the day‑to‑day smooth running of the service, receiving referrals and dealing with complex anticoagulation queries.
* Work face to face with service users, acting as the initial contact point and representing the Anticoagulation Team to users and stakeholders.
* Under the leadership of the Thrombosis and Anticoagulation Administration Manager, use independent judgement to determine when advice from senior colleagues is required.
* Follow all department protocols in dealings with colleagues and clients, contributing to and implementing changes when required.
* Raise concerns and escalation appropriately where unsatisfactory practice is identified.
* Work autonomously and in conjunction with the administrative team, clinical nurse specialists, Consultants and internal/external stakeholders, acting as a point of contact for patients/carers and dealing with enquiries from service users and other stakeholders.
* Attend and run point‑of‑care clinics for patients.
* Liaise between laboratory staff and service users in a polite and helpful manner.
* Attend meetings, including multi‑disciplinary meetings, ensuring effective communication and dissemination of relevant information.
* Provide anticoagulation referral reviews, add referrals onto DAWN database and CAREFLOW, and book clinic appointments.
* Identify and problem‑solve ongoing service issues and report to the Admin Manager.
* Close working with the Admin Manager to ensure constantly updated system reviews, patient management and best practice.
* Answer the telephone line, including managing the answer‑phone system, dealing with complex anticoagulation queries, and rebooking appointments as appropriate; pass on advice provided by the Clinical Nurse Specialist when required.
* Use the 4s DAWN Anticoagulation Dosing System to enter patient data accurately and promptly.
* Upload relevant documentation into DAWN, contributing to the development of a paperless environment.
* Perform quality control testing (NEQAS) for CoaguChek equipment as required.
* Act as a point of contact for patients, carers and telephone callers.
* Provide and receive complex and sensitive information such as personal medical information.
* Deal with enquiries from users, patients and other staff in a polite and helpful manner, seeking scientific staff advice where required.
* Work as part of a team and liaise with Medical Laboratory Assistants, Biomedical Scientists within the Haematology and Pathology Departments.
* Work alongside Trust colleagues to contribute to the Trust vision “We Care” and to contribute fully towards UHSussex NHS Trust corporate objectives, divisional and departmental delivery plans; demonstrate respect, dignity and privacy of all staff and patients and act with kindness and courtesy.
* Attend regular staff meetings, including multi‑disciplinary meetings, to ensure effective communication and dissemination of relevant information.
* Identify and problem‑solve ongoing service issues and report to the Administration Manager and/or Clinical Nurse Specialists as appropriate.
* Liaise closely with the Administration Manager, Service Lead and Clinical Nurse Specialist team, highlighting any issues in a timely manner.
* Ensure all clinical responsibilities are conducted according to Trust policies and guidelines regarding infection control and prevention.
* Contribute to provision of a safe and therapeutic environment for all staff, patients, relatives, friends and hospital visitors.
* Book patient clinic appointments using CAREFLOW.
* Adapt own practice in line with changes and developments of the service.
* Contribute as a team member, working a team to ensure the safe and smooth delivery for our patients, colleagues and service as a whole.
* Ability to work autonomously where required, problem‑solve and manage different areas of mixed workloads.
* Friendly and approachable to colleagues, patients, internal and external stakeholders.
* Excellent communicator both verbally and written; take personal responsibility for ensuring patients and staff are treated with courtesy and respect and confidentiality is maintained at all times.
* Act in ways that always follow the Trust's values by supporting people’s equality, diversity and rights.
* Ensure due regard is given to customs, values and spiritual beliefs of patients and carers.
* Report any concerns regarding patient care to the Administration Manager and/or the Clinical Nurse Specialists.
* Have a duty and responsibility to protect and safeguard adults and children, in accordance with national guidelines and Trust policies.
* Report any concerns relating to self or others regarding Health, Safety and Security to the Administration Manager and/or Clinical Nurse specialists.
* Comply with policies, procedures and clinical guidelines for self and others.
* Exhibit a high level of professional behaviour and attitudes at all times and demonstrate excellent customer care skills.
