About the Role/Responsibilities: This is an opportunity to join a newly established Birth to Settled Adulthood Service, working with young people and their families to support them as they approach adulthood. You will be part of a full time team working 37 hours, in a strength based multi-disciplinary way across health, social care, SEND and education. The role has a focus on preparation for adulthood from the age of 14 to 25 years. You will be completing Care Act assessments and care and support plans for young people along with other associated activities, such as completing MCA assessments/ Best interest meetings. You will also take a role in supporting colleagues with all aspects of preparation for adulthood work such as developing independence across all ages.
