The Reablement at Home Team provide short term re-ablementto people out in the community who need to regain their confidence and skillsfollowing discharge from hospital or maybe after a fall or unforeseen healthissues. Our service operates 7 days aweek, 365 days a year working in liaison with other healthcare professionals tosupport those who are in most need, to get them back to living as independentlyas possible.

Main duties of the job

Job title: Integrated Support Worker

Full and part time available on a week on week offrotapattern between the hours of7am and 10pm, including weekends and bank holidays

Permanent

The Integrated Support Worker is a key role in the delivery of our quality service and as such we provide an extensive induction and training programme to ensure you feel empowered to deliver an effective, safe and enabling service to our service users.

You will work in partnership with physiotherapists, occupational therapists, nurses and care managers to enable service users to achieve their best quality of life. You will support service users identified through an assessment process and act upon initial referral information responding flexibly to the patient/clients presenting needs. You will also be required to administer medication in accordance with Policy.

You will also benefit from a comprehensive benefits package including generous mileage allowance, annual leave entitlements, Saturday and Sunday unsocial hours payments and pension scheme.

You will have a NVQ 2/QCF in Health and Social Care or equivalent qualification or willing to work towards this as well as a standard level of literacy and numeracy skills and a basic level of computer skills including Microsoft Word.

Person Specification


Qualifications

* NVQ2/QCF in Health and Social Care or equivalent qualification or working towards it
* Care Certificate


Skills & Knowledge

* Knowledge of person-centred care
* Ability to maintain confidentiality at all times
* Knows when to consult others for help, advice and guidance
* Essential standard literacy/numeracy skills
* Ability to maintain accurate and legible records
* Be able to demonstrate empathy, tact and diplomacy particularly when faced with difficult situations


Other

* Ability to demonstrate commitment to CPG core values
* Ability to work weekends and Bank Holiday which will be part of an agreed rota pattern, however this can be changed at short notice to meet the needs of the service
* Able to work between the hours of 7.00am 10.00pm Monday to Sunday.
* Ability to transport self, and often equipment, within the boundaries of North East Lincolnshire


Experience

* Team worker with an appreciation of others skills and expertise
* Commitment to promoting independence for service users/individuals
* Is able to assist service users/individuals with social, personal, emotional needs whilst supporting with daily living skills
* Previous experience of working in a health and/or social care environment
* Previous personal experience of health and/or social care support
* Understanding of promoting independence and re-ablement
