Job Summary:
Partner Success Manager – Remote Care (Clinic Integration/Provider-Facing)
We seek a Partner Success Manager to oversee the clinical and operational success of our Remote Care programs. You will support provider groups and clinics in adopting Remote Patient Monitoring (RPM) to enhance chronic care management, patient engagement, and reimbursement models.
Your role involves acting as a liaison between partners and internal teams, guiding onboarding, workflow alignment, utilization tracking, and collaboration to ensure seamless service integration. Success metrics include clinic adoption, patient enrollment, quality and revenue outcomes, and partner engagement.
Responsibilities include:
* Managing clinic and partner relationships, understanding workflows, and providing training on RPM and billing compliance.
* Leading onboarding and go-live processes for new partners, facilitating EMR/EHR integration, and tracking performance metrics.
* Maintaining success plans, analyzing performance trends, and ensuring compliance with RPM standards and HIPAA regulations.
Qualifications:
* Bachelor’s degree in Healthcare Administration, Public Health, Nursing, Business, or related field.
* 2–4 years supporting provider groups, clinic operations, or in digital health/partner success roles.
* Knowledge of RPM, CCM, EHR/EMR systems, CRM tools, and performance dashboards.
* Strong communication, problem-solving, and relationship-building skills.
* Experience with healthcare technology adoption, CPT coding, and value-based care programs.
Work Environment & Schedule:
* Hybrid role based in Derby, CT, with 3–4 days/week onsite at clinics or in-office.
* Regional travel required for training and relationship building.

Additional Information:
* This role requires onsite presence in clinics/hospital or in-office weekly, with no relocation or visa sponsorship available. Candidates must reside within commuting distance of Derby, CT.
