3139 - Discharge and Patient Flow Matron
An opportunity has arisen in the Patient Flow and DischargeTeam to recruit a Discharge Matron to support the Discharge and Patient Flowteams.
Effective patient flow and timely and safe discharge iscrucial to patients and families and in supporting the Trust in maintainingquality and performance standards.
The successful post holder will be dynamic and supportdischarge and flow initiatives and processes to ensure patients at BHNFTexperience the best possible care in terms of placement and discharge. Theywill support the Nurse Manager for Patient Flow in managing the existing teamsand help to educate and support the wider teams in terms of discharge.
The successful candidate will be expected to support theimplementation of the new patient flow system (TeleTracking) and continue withthe initiative moving forward.
Main duties of the job
The candidate will maintain ahigh standard of work and lead and support discharges processes throughout theTrust.
They will support theDischarge Team and CBUs in ensuring that patient discharge is safe, timely andeffective and highlight areas that can be improved to support this.
They will work closely withpartners to ensure the highest possible standards of care in terms of dischargefor the Barnsley population.
They will lead and support theimplementation of the new Patient Flow System (Teletracking).

Qualifications
* 1st level Registered Nurse
* Demonstrate a current in depth knowledge of the national nursing agenda
* Teaching qualification or demonstrable evidence of equivalent relevant experience
* Educated to degree level
* Masters level qualification in health related subject or demonstrates knowledge and practice at masters level through evidence of experience in senior nursing role
