Overview

The role of Community Nurse is to work in partnership with service users in the designated care group with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives. They will demonstrate an understanding of clinical interventions relevant to the care group, including education, care planning and discharge planning, and will work closely with carers, families and partner agencies to facilitate this. The post holder will work to promote the safety and well-being of service users and their families/carers. They will assess, plan and implement care, provide specialist nursing advice and carry out specialist nursing procedures. They will provide clinical supervision to staff and students. The organisation provides mental health and learning disability care for children, young people and adults across Sussex and for children and young people in Hampshire. All clinicians are given opportunities to develop and grow, with regular supervision, a supportive working environment and access to training opportunities to realise potential.


Responsibilities

* Contribute to the management of a team caseload of clients with complex needs, using CPA principles, evidence-based/client-centred principles to assess, plan, implement and evaluate interventions, delivered in the Dementia and Older Adults Mental Health Service.
* Support/manage a team caseload and attend relevant MDT meetings.
* Regularly support and cover the duty rota.
* Provide specialist assessment and intervention as part of a multi-disciplinary approach, with specific responsibilities regarding the care pathway.
* Provide risk screening and assessment as well as risk management planning.
* Contribute to and facilitate evidence-based assessments and individual and group interventions.
* Contribute to the maintenance and development of the Service.
* Regularly share and supervise students on practice placement, and junior staff as appropriate.


Qualifications and Experience

* Registered Nurse with current registration (mental health/LD/Adult), appropriate to the job role.
* Completion of Mentorship Course/ENB equivalent.
* Evidence of continuous professional development; significant post-qualifying experience in the relevant care group.
* Experience of working alongside people with complex health problems in the designated care group.
* Experience of supervising staff.
* Experience of providing a range of clinical interventions to people in the designated care group with various health problems.
* Experience of working consultatively with professionals.
* Experience of conducting clinical assessments including risk assessments.
* Experience of partnership working within and across various statutory and non-statutory teams and agencies.
* Experience of working within the CPA process and understanding of clinical governance.
* Previous experience of working in the community. Appropriate UK professional registration is required.

Additional Information

We are a Trust committed to safeguarding children and vulnerable adults and ensuring staff complete appropriate training and procedures. The service operates 0900 - 1700 hours, Monday to Friday.
