Our Values One Team Accountability & Responsibility Quality & Innovation Relationships High Support, High Challenge About the Role We are seeking a committed and passionate Personal Advisor to join our 18+ Care Leavers Service, supporting young people aged 18–25 as they transition from care into independent adulthood. You will manage a caseload of care leavers, providing statutory support in line with legislation and guidance. This includes coordinating and completing Pathway Plans, advocating for young people, and ensuring they are supported with accommodation, education, employment, health, and emotional wellbeing. Key Responsibilities Manage a caseload of eligible, relevant, former relevant, and qualifying care leavers Prepare, implement, and review Pathway Plans in partnership with young people and professionals Support young people to develop independent living skills including budgeting and life skills Work collaboratively with partner agencies such as housing, education, employment, health, DWP, ...
