The service is made up of five teams: Enablement Team, 0-11 Team, 11+ Team, Eligibility Team and Occupational Therapy Team. The 0-11 team specialises in delivering early intervention in collaboration with children, their families and key agencies, including health and education. we are seeking an experienced social worker to undertake eligibility assessments for on going support and intervention from the disabled children service, this will be a non case holding role Key Objectives To participate in effective partnership working and engagement and to work collaborativelywith a range of organisations, statutory and voluntary in delivering services to children, youngpeople and adults.To work in co-production with children, young people, adults, their carers and signifcantothers in undertaking assessments and planning;To complete written records, reports and assessments to a good standard as required in linewith Children’s and Adult’s Services policies and practices.To ensure that the views of Children and Young People are listened to as part of any processundertaken;Knowledge / Education:Essential DesirableAn appropriate qualification i.e Masters or Degree in Social Work or recognised predecessor equivalent such as DipSW, C.Q.S.WKnowledge of legislation underpinningCommunity Care / Children’s Services Appropriate CPEL credits and /or post qualifying awards to demonstrate postqualifying competence.Childrens Services PostsUnderstanding of how the division operatesand knowledge of policies/procedures;Management qualification, e.g. D.M.S.,C.M.S;Hold a professional developmentqualification e.g. Practice Teachers Award,PQ6, NVQ Assessor Award;A sound understanding of the Law relatingto Children and FamiliesAn understanding of child and adolescentdevelopmentAdult Social Worker PostsWelsh Language Level 2 to Level 5.
