Position:
An exciting opportunity has arisen for x2 Advanced Nurse Practitioner's based at East Ayrshire Community Hospital.
What you will do:
We are seeking a highly motivated, enthusiastic and experienced Advanced Nurse Practitioner (ANP) looking for a new challenge to join our Community ANP Team. Based at East Ayrshire Community Hospital. You will work collaboratively with the multidisciplinary team, support and promote early discharge to a homely setting; respond to existing or potential new ill health issues in the person’s home/homely setting; contribute to a reduction in unplanned acute hospital admissions and work within East Ayrshire Community Hospital to support care delivery by providing a variety of advanced practice care. The role will require you to provide advanced levels of autonomous nursing practice to undertake physical assessment, diagnose and plan care or onward referral for people in your care. Alongside the clinical daily decision making, leadership and continuity of care this post will bring, the post holder will also work closely with the enhanced integrated care team, ANPs and acute hospital colleagues to support smooth transitions of care to and from home and hospital.

Knowledge, training and/or experience required to do the job:
You must be registered nurse, a qualified Advanced Nurse Practitioner with MSc post grad qualification or equivalent in Advanced Practice or be able to evidence working at Advanced Practice Masters level. You will also have experience within an appropriate clinical environment and demonstrate experience of teaching/education of others.
