We work collaboratively with young adults throughout their transition from CAMHS to Adult Mental Health Services. You will be offering support, advice, and information to young adults as well as family and carers as they progress through to adult mental health services.
We want to ensure that young adults are supported and empowered to make decisions around their mental health and wellbeing.
The role will ensure that young adults moving from CAMHS services into adult services are sufficiently prepared and supported to navigate adult provision and networks, whilst maintaining their progress and promoting independence.

Main duties of the job
The aim of role is to support the successful transition of young adults aged 18-25 into appropriate mental health services and to enhance the social and mental health outcomes of young people experiencing difficulties as they grow into adulthood.
The Role Will Include:
* Building a therapeutic relationship with the young adult, listening to the young adult fully and helping them to explore their aspirations and concerns about the future
* Encouragement of responsibility for themselves, their mental health and wellbeing
* Supporting independence and strengthening their network
* Helping the young adult to get familiar with adult services (transport, location and people they need to know)
* Collaborative planning to manage their health and wellbeing
Helping the young adult to find solutions to some of their problems by completing brief interventions.
supporting the young adult, their carers and their CAMHS worker to make their transition run smoothly
* you will work collaboratively with the lead professional and care coordinator to support the delivery of the transition and care plan
* alert the care co-ordinator or key worker or allocated worker of any concerns regarding quality, safety or effectiveness of care, or if a young adult has not attended an appointment or there is concern that they are disengaging from services

The post holder will:
Undertake delegated duties under the supervision of registered health care professionals. Consulting as necessary and working collaboratively within the health care team to deliver therapeutic interventions, psychological and physical care to a defined group of service users.
deliver evidence based planned care, which is safe and effective, in a manner which is caring, compassionate and enables service users to reach an optimum level of functioning, independence, rehabilitation and recovery.
provide care within well established policies, procedures, protocols, guidelines, and established clinical care plans, whilst acting within the required sphere of clinical competences for the role at all times.
following intial assessment by a registered health care professional, exercise initiative and judgement in, care planning, goal setting, delivery, implementation and evaluation of care.
working collaboratively and in partnership with others, develop knowledge to promote holistic care.
demonstrate sound knowledge of the care and treatment programmes of individual service users toenable the post holder to contribute and inform the decision making of registered health care professionals.
make appropriate care adjustments, within the agreed level of autonomy for the post, and be able to communicate any changes to service users, carers and the health care team.
