We are seeking an experienced and resilient Band 6 Community Nurse to join the a Client Assessment Team in Telford. This role is central to the delivery of high-standard mental health care for Children and Young People (CYP). You will act as a Shift Coordinator, providing clinical expertise in the screening, assessment, and signposting of referrals within the Thrive Model framework.
Key Responsibilities

Shift Coordination & Triage: Screen incoming referrals to categorize urgency (Routine, Urgent, or Crisis).

Expert Consultation: Act as a point of contact for professionals, parents, and guardians regarding mental health concerns and risk management.

Clinical Assessment: Conduct comprehensive face-to-face assessments (both urgent and routine) for children and young people.
*
MDT Collaboration: Present assessment outcomes in daily Multi-Disciplinary Team (MDT) meetings to determine intervention pathways or discharge.

Documentation: Maintain high-standard clinical records within the operational framework’s timeframes.

Leadership: Provide essential supervision and mentorship to junior staff and students within the team.
Candidate Requirements

Registration: Valid NMC Registration (Mental Health Nursing preferred).
*
Experience: Previous experience in a CAMHS or community mental health setting at a Band 6 level.
*
Framework Knowledge: Familiarity with the Thrive Model of mental health care is highly desirable.

Skills: Strong risk assessment skills, professional curiosity, and the ability to remain calm under pressure during crisis calls
