GP Practice in Wales is seeking an experienced Practice Nurse to join our team in Conwy. You will provide excellent care to our patients, working collaboratively within a multi-disciplinary team to deliver health services.
Your responsibilities will include managing chronic diseases, providing immunisations, and supporting the clinical team on safeguarding matters. A diploma in chronic disease management and NMC registration are required.
The position is part-time, approximately 24 hours a week, and salary details are available upon request.
