Overview
Join a compassionate, progressive team focused on helping adults live healthier, connected, independent lives. Work is strengths‑based and preventative, using the Three Conversations approach. These roles will be within the Aging Well service.
Responsibilities
* Promote independence, wellbeing, and choice
* Work collaboratively with health, housing and community partners
* Champion the voice of adults and carers
* Support outcome‑focused, person‑centred practice
* Work directly with people with care and support needs and carers to ensure that their voices are heard, and they are supported to live their idea of a good life
* Use relevant legislation, social work models and theories to inform practice
* Apply analytical, assessment and negotiation skills
* Use strengths‑based and evidence‑based approaches in service delivery
* Employ strong interpersonal and communication skills
* Manage time effectively and use IT skills to support practice
Qualifications
* Social Work England registered (or in a position to be registered by September 2026)
* Social Work CPD / Career Pathway to support progress and development
