Job Title: Band 6 Sister / Charge Nurse - Discharge Team
The post holder will work in partnership with colleagues across health and social care, both in and out of hospital, to facilitate the safe discharge of patients with complex care and palliative needs from acute hospitals. The goal is to reduce length of stay and improve outcomes for patients and their carers.
Responsibilities include:
1. Coordinating and managing the discharge process for patients with complex healthcare needs, including complex continuing care assessments.
2. Providing specialist advice on community/primary care services to enable multidisciplinary teams to make informed risk assessments, supporting patient transfers and discharges.
3. Conducting holistic assessments of patients with complex needs and liaising with multidisciplinary ward teams for safe and timely discharges.
4. Maintaining strong collaborative working relationships with health/social care and other stakeholders.
5. Working closely with palliative care teams to facilitate fast-track discharges for patients in their last days or weeks of life.
6. Collaborating with the continuing healthcare team to apply for funding and support screening for continuing nursing care.

This role is open to individuals with a current NMC PIN to practice as a Registered Nurse or Midwife in the UK, or those enrolled in a course expecting to receive their full NMC PIN within 3 months. Please note that an NMC PRN PIN does not meet the essential criteria for this role.

You should demonstrate skills in multidisciplinary team working and the ability to work independently. You will provide specialist assessments to facilitate discharges to Intermediate Care, Palliative Care, or Discharge to Assess beds in residential and nursing care facilities.
