Job Description
We are currently seeking a Social Care Officer: CWD Short Breaks Coordinator to provide sickness cover for our short breaks service. This role is essential in assessing the eligibility and need for short breaks, securing the right provision to meet these needs, and ensuring continuous review in line with regulatory requirements. This position is ideal for someone with a strong background in working with children with disabilities and their families.
Day-to-day of the role:
* Work flexibly as part of the CWD short breaks team with an allocated caseload.
* Assess the eligibility and need for a short break and secure the appropriate provision to meet these needs.
* Ensure that the short breaks plan is reviewed regularly in line with regulatory requirements to confirm it meets the identified needs of the child and their family.
* Collaborate with partners across education, health, and care sectors, focusing on outcomes for children.
* Report directly to the Team Leader and work restoratively with children and their families.
Required Skills & Qualifications:
* Substantial experience working with children with disabilities and their families.
* Proven experience in conducting assessments (e.g., Early Help Assessments).
* Excellent communication and interpersonal skills.
* Ability to work collaboratively with various partners across different sectors.
* A restorative approach to working with children and their families.
