Introduction
The Home First service integrates nurses, paramedics, therapists, and health and therapy assistants to collaborate with the acute Multi-Disciplinary Team & Transfer of Care Hubs. The goal is to facilitate an efficient and effective discharge process from hospital to the patient's own home.
Our multidisciplinary teams operate flexibly, providing a 7-day service, 365 days a year, from 8am to 8pm. Patients discharged into Home First receive step-down intermediate care, which includes community-based assessments and interventions in their own homes. This support aims to promote recovery and monitor health post-discharge to prevent re-admission. When necessary, Home First offers acute reablement for up to 10 days to help individuals achieve functional goals and regain independence in daily activities.
The core principle of the service is enabling recovery by working towards maximum independence for each person.

Main Duties
As a Health and Therapy Assistant, you will provide personal, health, and therapy care to individuals across Sussex with diverse and acute needs. Your day will be varied and engaging, supporting people to increase their independence.
You will assist multidisciplinary colleagues with ongoing assessments and care provision as outlined in personalized care plans. This includes procedures such as taking physical observations, venipuncture, medication administration, wound care, rehabilitation programs, and equipment provision. You will observe changes in patients' conditions and communicate updates to your team through verbal reports and accurate record-keeping.
No prior experience is required; we will provide on-the-job training to develop your skills, including basic nursing and therapy techniques, and support you in achieving the Care Certificate. Our comprehensive competency framework will support your continuous professional development.
With experience and further training, you can advance your career in healthcare, potentially training as a Nursing Associate or progressing to degree-level healthcare roles such as Nurse, Podiatrist, Midwife, or Therapist.

Qualifications
* GCSE level education or equivalent
* NVQ Level 2 or equivalent in health and social care, or willingness to undertake
