Main duties of the job
As a Team Leader at Avery Healthcare, you will support the home management team in delivering high-quality, person-centred care. You will be responsible for implementing, monitoring, and evaluating care plans, assisting in smooth resident discharges, and ensuring the well-being of each resident. You will also lead and monitor staff, ensure safe systems of work, and promote a caring, supportive, and inclusive environment.

Job responsibilities
Other responsibilities will include:
* Leading in the person-centred care planning process by implementing, monitoring, undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
* Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping residents’ next of kin fully informed, where appropriate.
* Leading with monitoring the well-being of each resident, being aware of any change impacting care and care planning, and ensuring that changing needs are met and written records amended to reflect any variation.
* Practising safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.

Our ideal candidate must:
* Have an NVQ 3 or equivalent in care for care homes.
* Have two years’ experience working in a senior position with the resident group.
* Have the ability to communicate effectively both verbally and in writing.
* Demonstrate the ability to lead staff and act responsibly in the absence of the manager.

Qualifications
