Overview
This is an exciting secondment opportunity to work within the CHFT multi-disciplinary specialist respiratory team. The post holder will take the lead on attaining accreditation for the pulmonary rehabilitation service, working alongside the existing clinical lead physiotherapist. The team delivers a community pulmonary rehabilitation service that supports and promotes optimal self-care. The service may be delivered in class settings, through individually tailored home-based rehabilitation as part of a supported discharge pathway, or virtually using digital and remote platforms.
Responsibilities
* Act as a change agent and innovator, planning, instigating, and evaluating change in line with the pulmonary rehabilitation accreditation standards.
* Assist the established pulmonary rehabilitation team to implement standards to achieve accreditation, developing and maintaining collaborative relationships, partnerships, and networks within the local region.
* Review current provision of the service and manage the process of meeting the established standards needed to become an accredited service, including a small element of clinical caseload, running groups, and supervision of less senior team members in the absence of the clinical lead.
* Contribute to measuring the effectiveness and productivity of pulmonary rehabilitation services through data collection for the team annual report and ongoing national audits.
* Support the pulmonary rehabilitation clinical lead in the daily operational management of pulmonary rehabilitation and manage a small clinical workload of patients with chronic respiratory disease at a highly specialist level, assessing and treating patients and maintaining clinical records.
* Undertake comprehensive patient assessments (including complex presentations), formulate individualised management plans, and deliver evidence-based physiotherapy/pulmonary rehabilitation treatment programs with high autonomy.
* Develop and deliver appropriate treatment programs, often choosing from a range of options and evaluating the effectiveness of interventions.
* Develop effective links with community and specialist respiratory services (including GPs, Practice Nurses, Respiratory Physicians, mental health services, and Community Pharmacists) and provide specialist advice or referrals as required.
* Provide education and training to Physiotherapists and to qualified and non-qualified healthcare, social services, education staff, and medical staff.
* Liaise with respiratory nurses to avoid admission/readmission and support a swift return to independence.
* Work autonomously within Trust policies and professional practice, maintaining accurate and up-to-date records and adhering to risk management policies.
* Attend induction and regular update training on risk management policies and procedures; identify risks within the department or community during service delivery; report incidents and assist in management of complaints and incidents.
* Develop and maintain collaborative relationships, partnerships, and networks to influence and improve the delivery of pulmonary rehabilitation.
* Support the clinical lead in the daily operational management of pulmonary rehabilitation and contribute to achieving service objectives.
Qualifications and Experience
* Experience in project management is useful; previous experience in the respiratory field, particularly pulmonary rehabilitation, is essential.
* Knowledge of the PRSAS is an advantage.

* Develop and maintain collaborative relationships to influence health outcomes and health delivery systems.
* Ensure service developments for chronic respiratory disease improve practice and align with local and national guidance.
