Job Description
* Dispensing and educating patients on their medications, medication review and reconciliation, therapeutic interventions and recommendations, multidisciplinary rounds and collaborative prescribing for qualified persons.
* Recommend and monitor drug therapy to achieve desired outcomes.
* Provide drug information to other healthcare professional and patients.
* Identify, resolve and prevent drug-related problems.
* Provide pharmaceutical care, working collaboratively in a multidisciplinary team in various clinical settings, including outpatient, inpatient and community settings.
* Ensure pharmacy policies and procedures are implemented consistently.
Requirements
* Degree in Pharmacy
