What you will do:
* Develop and maintain long-term, supportive relationships with children with disabilities and their families.
* Coordinate and deliver social care services for children who are subject to Child in Need or Child Protection plans.
* Prepare for and participate in court proceedings and multi-agency planning meetings.
* Carry out comprehensive assessments, advocating effectively for the needs and wellbeing of children and families.
* Work collaboratively with a supportive and compassionate multidisciplinary team to develop creative and effective care solutions.
