Main duties of the job
Youwill be responsible for the delivery and management of long term conditions, health education, treatment, assessment and screening the practice population whilst working collaboratively with the practice team, PCN team and community teams.

Job responsibilities
Main Purpose
The practice nurse willwork as part of the primary healthcare team providing General Medical Servicesto the patients of Practice.
Providingassessment, screening and treatment services and health education advice
Provide nursing treatments to patients in participation withgeneral practitioners or independently to agreed protocols.
Provide general and specific health screenings to the practicepatients (within agreed protocols) with referral to general practitioners asnecessary.
Advise patients on general health care and minor ailments withreferral to GPs as necessary.
Supplies and equipment Treatment room and other areas
In participation with the practice manager, ensure themaintenance of equipment and stock levels relating to patient care.
Pathologicalspecimens and investigatory procedures
Undertake the collection, storage and despatch of pathologicalspecimens including intravenous blood samples, swabs, smears etc and ensure thecleanliness of any equipment so used.Perform any investigatory procedures requested by the GPs.
Administrativeand professional responsibilities
Participate in the administrative and professionalresponsibilities of the practice team.
Ensure accurate notes of all consultations and treatments arerecorded in the patients notes on the clinical computer system as appropriate.
Ensure accurate completion of all necessary documentationassociated with patient health care and registration with the practice.
Ensure collection and maintenance of statistical informationrequired for regular and ad hoc reports.
Attend and participate in practice meetings as required.
Assist in the formulation of practice philosophy, strategy andpolicy and develop appropriate protocols.
Researchprojects
Co-operate and participate as required in any research projectswithin the practice
Student/staff education and training
Participate in the education and training of students of alldisciplines and the induction of all members of practice staff with specialresponsibility for nursing staff.

Maintaining and cleaning equipment used by the nurses and GPs

Ordering of Health Promotion Literature
Ordering of stock from the Hospital
Ordering of vaccinations to maintain stock levels
Participation in administrative systems in the practice
At all times there will be a need to maintain accurate records
A duty to advise GPs/Practice Manager of potential problemsor errors within the range of assigned tasks
Attend and participate in any practice meetings when required.
Undertake home visits
