Emotional Health and Wellbeing Nurse - Care Leavers
The post holder will work collaboratively with Luton Borough Council, ELFT Adult Mental Health Service and CAMHS. The post holder will be attached to the Luton Community Mental Health Service working directly with Care Leavers (18-21 years, with the view to 18-25 yr after the post has been embedded).
The post holder will be a highly specialist and experienced Nurse with a substantial knowledge in mental health, including child and adolescent mental health (CAMHS). The role involves supporting young people’s physical, emotional and mental health needs in preparation for transition into adulthood and independence.
Responsibilities
* Work directly with Care Leavers in the holistic assessment of health needs, supporting transition into adult services, sign‑posting and referral onto appropriate agencies.
* Communicate effectively with young people, including those who have been hard to reach, using a patient and empowering approach.
* Provide one‑to‑one support for young people with emotional health needs and sign‑post to specialist services.
* Provide regular health and well‑being advice at the Luton Care Leavers Hub, including face‑to‑face sessions and the use of approved social media technology (video, WhatsApp, messaging).
* Support Care Leavers to make behaviour changes that lead to positive lifestyle changes.
* Work with local services to meet the needs of Care Leavers with special educational needs and disabilities.
* Support unaccompanied asylum‑seeking children to access trauma‑based therapies and prepare them for independent living.
* Attend the London Regional Looked After Children Forum.
* Link with the Luton Borough Council Care Leavers Forum / Participation Team.
* Act as a resource of specialist advice on care‑experienced teenagers for colleagues in CAMHS, other professionals and agencies.
* Liaise with and participate in joint work with the Clinical Nurse Specialists in Youth Justice, Justice and Substance Misuse services, and the Trust adolescent and adult inpatient services.
* Liaise with adult mental health and adolescent inpatient services when appropriate.
* Ensure the client group's needs are paramount and adhere to local child protection procedures and safeguarding policies.
* Maintain acceptable safety levels for self and advise colleagues when necessary; adhere to lone worker policy and update training in breakaway techniques.
* Travel to young people placed both within and outside the borough, using public transport or own vehicle in line with ELFT policy.
* Link with Luton Virtual School to support access to education, training or employment.

* Qualified Nurse registered with the NMC
Experience of Transitions work
* 2 years experience post qualifying
