Role Responsibilities: Undertake varied casework including support planning, commissioning care, conducting reviews, and mental capacity assessments. Facilitate solution-focused, strengths-based conversations, including Conversation 3 discussions to assist young people transitioning to adult services. Participate in multi-disciplinary meetings, including best interest meetings, EHCP reviews, Section 117 reviews, and CHC checklist assessments. Work closely with senior practitioners and team managers, drawing upon their support and professional guidance. Act as a dedicated advocate for young adults, ensuring their voice, wellbeing, and goals are prioritised in every care decision. Candidate Requirements: A minimum of 2 years' post-qualification experience in social care, ideally within a transitions or related service. Strong case management skills and the ability to work effectively in a fast-paced, evolving environment.
