Golden Hello payment £1,000 + £5,000 per annum

Are you a passionate and qualified Social Worker looking to make a real difference in urgent care settings? Join our dynamic ADAPT Team, a frontline service based at the Midland Metropolitan University Hospital (formerly City Hospital), supporting citizens from Birmingham’s most deprived communities.

The ADAPT (Admission Avoidance and Discharge Planning Team) is a front-door, integrated health and social care service operating 7 days a week. We work closely with Emergency Departments and medical assessment units to prevent unnecessary hospital admissions and support timely, safe discharges.

Our team plays a vital role in:

* Supporting patient flow across the hospital system
* Identifying community-based solutions
* Signposting and advising citizens and carers
* Conducting Care Act and Mental Capacity assessments


What We’re Looking For

* Hold a CQSW, DipSW, CSS or equivalent Social Work England-recognised qualification
* Registered with Social Work England
* Have experience working with vulnerable adults (hospital experience is desirable but not essential)
* Can work effectively in a fast-paced, multidisciplinary environment
* Is confident in making decisions under pressure and managing complex needs
* Is committed to equality, continual professional development, and service improvement
