Interested in joining Routes as a Case Manager in the Merseyside/Warrington area? Here’s what we’re looking for:
* A passion for putting people first.
* Strong organizational and time management skills.
* A good understanding of CQC requirements.
* Experience managing complex care packages with clinical interventions (e.g., spinal injuries, acquired brain injuries, motor neuron disease, multiple sclerosis).
* Experience leading a team of healthcare workers.
* You must be a driver with your own car.
Your core responsibilities will include:
* Managing a caseload of clients with complex needs, including clinical interventions, and overseeing their care holistically.
* Coordinating and leading the transition, setup, and ongoing management of care packages, involving clients and multidisciplinary teams.
* Maintaining high standards of care and a person-centred approach.
* Prioritizing individual wellbeing.
* Acting as an advocate for complaints and safeguarding, and promoting continuous improvement.
* Developing care plans and risk assessments.
* Providing support through observations, mentoring, and regular reviews.
