The Orpington Primary Care Network is a group of general practice surgeries in the outer London suburbs. We currently have twenty eight staff including a team of eight clinical pharmacists and four pharmacy technicians led by a Lead Clinical Pharmacist and are looking to expand the team. We are looking for a 0.6WTE part time pharmacist.
Main duties of the job
* Provide professional advice and support on the safe prescribing, handling and administration of medicines. This includes support regarding development and implementation of medicines management processes and policies.
* Lead on the medicines aspects relating to the care of patients to optimise medicines, reduce medicines-related risks and improve adherence to taking medicines by carrying out medication assessments and reviews in identified patients.
* Resolve day-to-day medicines and prescription issues with healthcare professionals
* Help patients to manage their conditions
* Lead repeat prescription services and deal with queries from staff and patients
* Present to patients any changes to prescribing in line with NICE, best practice or in response to prescribing alerts
* Advise patients of the adverse side effect of medication and potential interaction with other medication/treatments
* Rationalise repeat prescriptions lists to avoid waste and duplication
* Provide reviews to those patients with LTCs or complex health needs, including drug monitoring and risk versus benefit.
* Provide both face to face and digital appointments to practice patients
* Contribute to the management of the prescribing budget through the promotion of safe, cost effective rational prescribing
* Contribute to minimising the harms associated with the use of medicines, for example by deprescribing

Job responsibilities
An Orpington PCN pharmacist will provide clinical care drawing on a background in pharmacy. You will be the medicines expert within your surgerys team, and work within a multidisciplinary mix of healthcare professionals from GPs and nurses, to mental health practitioners and paramedics. You will lead the way for patient-centred medication reviews, drug safety, and cost-effective and efficient prescribing within primary care. You will be supported by the rest of our pharmacy team and GPs to fulfil your full potential as a pharmacist.

Qualifications
* Current UK professional registration with the General Pharmaceutical Council.
* Postgraduate qualification in clinical/community pharmacy or equivalent additional experience.
* Completed CPPE
