The highly performing team provides the social work role within Derbyshire Community Health Services (DCHS) Community First service. The team is based at the Royal Derby Hospital, however Hospital to Home also work with customers across local Community Hospitals, their own homes and within Perth House assessment centre. Your customer base is people over the age of 18 years who require advice, advocacy and support to maximise their independence, safety and well being. The team works closely with the Integrated discharge team and DCHS to discharge customers using the discharge to assess model. Detailed job description and main responsibilities To provide Social Work and Care Management services to patients within Acute hospital trusts and Derbyshire Community Health Services discharge pathways. DCHS provides Care Act 2014 functions on behalf of Derby City Council under a Section 75 agreement. Social workers provide high-quality social work support to patients and their families, ensuring that individuals receive the appropriate care, advice, and assistance during their hospital stay and as they transition back home. Social workers are involved in the development of services and staff. They co-ordinate the assessment of needs of patients and work with them and their carers in accordance with agreed multi-agency assessment processes and hospital discharge procedures. Social workers will promote the discharge to assess model to meet Care Act 14 eligible needs. They will ensure that assessments are completed and arrange or commission the provision of services appropriate to meet the assessed needs. Social workers work with the trust integrated discharge team to prevent delayed transfers of care. They require a knowledge of a wide range of services and other resources, including statutory, voluntary, and private provision. Social workers assist the Team Manager and Social Work team leads in the development of policy and practice standards.
