Job Overview
An exciting opportunity has arisen for a Community Staff Nurse to join the Brighton & Hove Community PCN, Central Team.
The PCN is a group of practices working together in collaboration with community and other health and care services. PCNs enable greater provision of proactive, personalised, coordinated, and integrated health and social care, organized around individuals' needs through collaborative working to deliver excellent patient care at the community's heart.
Brighton & Hove Community PCN Central Team is looking for registered nurses. Whether newly registered or seeking a new challenge, community nursing offers a diverse and rewarding specialty.
All successful applicants will receive an individual induction plan to support their transition into this community environment.
Role Responsibilities
* Deliver high-quality care in patients' homes, supporting rehabilitation, self-management, and independence.
* Participate in a team supporting evidence-based care, staff learning, and development.
* Manage conditions such as diabetes, wounds (including leg ulcers), palliative care, bladder and bowel care, and continence needs.
* Work with a specific group of patients under frailty, including those with dementia, end-of-life care, complex conditions, and care home residents.
